2026 T2G 7v7 Registration Form

* I hereby, grant permission for my child to participate in any and all True 2 The Grind program(s), events(s), including but not limited to, athletic, social, and/or fundraising activities. I further consent to the administration of any and all medical treatment necessary to stabilize and/or treat any medical condition or medical emergency to which my child is afflicted. I understand that this authorization is given prior to the need for medical care but given in advanced to avoid any uneccessary delay in emergency treatment which the attendant and/or medical professional may deem advisable in the exercise of their best judgement. 

* I acknowledge that I am fully aware of the potential dangers of participation in football, may result in SERIOUS INJURIES, PARALYSIS, PERMANENT DISABILITY AND/OR DEATH. Furthermore, I fully acknowledge and understand that protective equipment does not prevent all participant injuries. I, do hereby, give my approval for my child to participate, and further assert that I have verified with my child's physician, and in my opinion, my child is physically fit and can participate without limitation in any and all Local, Regional, National, League/Conference, Association and team/squad activities including transportation to and from the activities by a licensed driver.

Athlete's Conduct:

I will: Emphasis the ideals of sportsmanship, ethical conduct, and fair play. Show courtesy to my opponents and officials. Recognize athletic contests are serius educational endeavors. Give complete allegiance to my coaches who are the instructional authority for my team. Discourage fans, fellow players, and parents undercutting my coache's authority.

I will not: use profanity or "talk trash" before, during or after any games. Use drugs, alcohol or tobacco. Criticize my team mates. Act in a way that will incite spectators.

Parent's Conduct:

I will: Support my child's team/squad and teach the value of commitment to the team/squad- empahsis the ideals of sportsmanship, ethical conduct  and fair play. Help my child and True 2 The Grind make athletic contests a positive educational expericence. Show courtesy to opponents and to officials. Direct constructive criticism of my child's athletic program to the athletic director or association officials and work towards a positive result for all concerned. 

I will not: Criticize officials, direct abuse or profane language toward them, or otherwise subvert their authority. Undermine, in work or deed, the authority of the coach or administration. Intrude onto the field , stand on the sideline, or yell from the bleachers at or to the coaches, referees or administration.

Medical and Liability Release (must be accepted):

In consideration of the acceptance of my application for entry into the above event/activity, I hereby waive, release and discharge any and all claims for damages which my child may have or which hereafter accrue to him/her against True 2 The Grind and it’s affiliates, partners, directors, advisors, volunteers as a results of his/her participation in the event/activity. This includes any of my family members, friends and I onsite. I hereby waive, release and discharge True 2 The Grind. I agree to view from authorized areas and stay off all equipment and from underneath bleachers.

I hereby waive, release and discharge any and all claims for damages/injury which my child may endure during any event/activity with True 2 The Grind and it’s affiliates, partners, directors, advisors and volunteers.

I hereby release, discharge and or otherwise indemnify True 2 The Grind and it’s affiliated organizations and sponsors, their employees and associated personnel, including the owners ofthe fields and facilities utilized for event/activity, against any claims by or on behalf of my child as a result of child’s participation in the event/activity. 

Photo/Video Use Grant:

I authorize True 2 The Grind and it’s affiliates/partners to use photographic and videography images of my child for marketing communications, websites and social media.

Safety:

Mouthpiece, cleats and softshell helmets are required to play. Parent/Guardian is resposible to ensure their child has all items during a game.

Refund Policy:

NO REFUNDS

I accept the waiver, photo/video use grant, safety and refund policy.

Student Athlete/Parent Contract 2025-2026:

I have read, understand and agree to abide by the guidelines set forth below. I understand that participating in the program is a privilege and responsibility. It is a privilege to develop various athletic skills and a responsibility to represent the program with your best sportsmanship and attitude.

Athlete:

I agree to:

- Maintain a 3.0 GPA (High School)

- Attend practices. Practices are mandatory!

- Fully commit to the program. Playing for another team will result in being suspended and/or dismissed from program

- Participate in all scheduled tournaments. More than 2 missed tournaments may result in being suspended and/or dismissed from the program

- Demonstrate good sportsmanship at all times 

- Be a positive representative of our program 

- Respect other players and coaches

Parent:

I agree to:

- Cannot be on the field during games

- Contact a coach or board member before practice or any tournament if athlete cannot attend 

- Report any personal injury to a coach immediately

- Turn in all the necessary forms issued to the coach

- Demonstrate positive support for all players and coaches at practices and tournaments 

Registration:

For a complete registration, I agree that I must:

(1) Complete the entire registration form for my child

(2) Enroll in the 3-month installment plan

Installment plan terms:

12u - $167 will be deducted per month for 3 months

14u/HS - $267 will be deducted per month for 3 months

Transaction fees and a one-time set-up fee will be apply.

1st month's payment will be on your registration date.

I agree to the installment plan terms. I understand if I fail to enroll in the 3-month installment plan, my child's registration will be considered incomplete until all steps are completed. I understand my child will not be provided a jersey or sweatsuit if payment is not received. I understand my child may be removed from the team if payment is not received. I acknowledge all payments are non-refundable.

We have read and will adhere to the student/parent contract while being a player/parent for True 2 The Grind. I agree to the registration and installment plan terms. Failure to abide by this contract will result in being dismissed from the program.
